If you are a new patient, Prairie Lakes Healthcare System will send you an email to the email address you provide that includes instructions and a link to register and access your MyCareCorner account.

Existing Thrive Patient Portal Users
If you have a registered account on the previous Thrive patient portal, you will need to login to your old patient portal account at thrivepatientportal.com with your existing Username and Password. You will be redirected to the MyCareCorner login/registration page. Fill out the appropriate fields to complete registration for your MyCareCorner account.
Download the MyCareCorner Mobile App
The MyCareCorner App provides mobile access to the health information within your MyCareCorner account allowing you to track and access certain medical information right from your mobile device.
If you have forgotten your password, select the “forgot password” option on the registration screen, answer security questions, and reset your password.
If you forgot your userID or are having any issues logging in or navigating the patient portal, you may reach Prairie Lakes Healthcare System staff by submitting an assistance request (see button below) or by calling 605-882-7620 or toll-free at 855-215-5323 during normal business hours.
