During the month of November, the home care and hospice community honors the millions of nurses, home care aides, therapists, social workers, and support staff nation-wide who make a remarkable difference for the patients and families they serve.  These heroic caregivers play a central role in our health care system and in homes across the nation. 

Prairie Lakes Home Care

  • Provides skilled and non-skilled home health services to over 300 patients
  • Counties Served: Clark, Codington, Day, Deuel, Grant, Hamlin, and Roberts.

Prairie Lakes Hospice

  • Serves an average of 30 patients daily across this same region, including 13 nursing homes and assisted living centers.
  • Counties Served: Clark, Codington, Day, Deuel, Grant, Hamlin, and Roberts.

To cover this expansive region, we employ:

  • 25 nurses
  • 16 homecare aides
  • 5 physical therapists and PT assistants
  • 5 occupational therapists and OT assistants
  • 2 speech therapists
  • 2 social workers
  • a spiritual care coordinator.
  • Billing and support staff
